2023 Tentative Football Schedule

Important Information to Note:

• All equipment is provided by J. C. Booth Middle School (helmets, shoulder pads, hip/butt/thigh and knee pads, practice pants, practice jersey, mouthpiece, and game uniforms).  All you have to provide are cleats and athletic shoes (in case of rain).
• We typically use REMIND to push out information. To sign up for REMIND, please click here.  Please feel free to email me with any additional questions you may have. I will push out Summer Workouts through Remind.
• We are looking for someone with the ability to tape our games. If this is something you are interested in, please email Coach Mothershed at mothershed.nate@fcboe.org.  Any help with this would be greatly appreciated.
• Summer Workouts: Please be sure you have turned in your Physical, Insurance, and Consent (including a copy of the front and back of your insurance card), Heat and Concussion, and Cardiac Awareness forms. Student-athletes will not be able to participate in summer workouts without all the mandatory paperwork turned in.

All athletes will need to have all forms completed and turned in on July 24, 2023. The link to all forms needed is below.

Mandatory Paperwork to Participate in Football

The following forms and documents need to be completed and submitted:

• Physical
• Parental Consent
• Permission to Participate
• Code of Conduct
• Concussion Fact Sheet
• Concussion Awareness
• Cardiac Arrest
• Copy of Insurance Card front and back

If you do not have insurance, you will need to purchase school insurance. You can get that form at the front office.
