New Student Registration

Parents will need to bring the following information:

*Certified Copy of the child’s birth certificate (with state seal and number)

*Copy of child’s Social Security card

*Immunization record on a Georgia form (#3231)

*Eye, Ear, and Dental record on a Georgia form (#3300)

*Current gas or electric bill – telephone or water bill is not acceptable

*Current original signed lease agreement OR if parent owns home, a copy of warranty/security deed. If it is a recently purchased home (within the last 30 days), we can accept signed settlement papers.

If a parent is living with someone in our district and is not a legal resident, he/she must contact the school at
770-460-3565 to obtain an appointment for an Affidavit.
