Student/Parent Forms for Rank One

Fayette County Public Schools uses Rank One for managing physicals and all required parental permission forms. If your student does not already have a Rank One profile for your currently enrolled school, please complete the Athletic Interest Survey in Infinite Campus (directions can be found here). A student profile will be created in Rank One within 24-72 hours after completing this form.
