Transcript Requests

The Enrollment and Records Center has records dating back to the 1940's. Transcripts can be provided for graduates and students that have withdrawn from Fayette County Public Schools. 

Process:

• A signed Transcript Request Form and photo ID are required before a request is processed. The fee is $5 per transcript, which may be paid with cash or money order only. We can not accept personal checks.
  
  
  
• A Transcript Request Form may be downloaded and mailed to the address below or you may come to the center for immediate service.

Mail Your Completed Transcript Request along with $5 for each transcript to:
