  • Absence Excuse Information

    Please submit an excuse request one of the following ways:

    2. Infinite Campus Request

      • Week Day(s) Absent
      • Reason for Absence
      • Parent/Legal Guardian Signature
      • Current Phone Number

    Excuse notes from parent/doctor must be turned in to the attendance office within 3 days of the absence for the absence to be excused.  Students are limited to 5 parent notes per semester, tardy notes are included.

    Student Driver Self-Checkout

    Please submit a request one of the following ways:

    1. Check Out Request Form
    2. Paper Request Must Include:
      • Student's Full Name
      • Date & Time of Check Out/Check In
      • Parent/Legal Guardian Signature
      • Current Phone Number

    We must have verbal confirmation from a parent/guardian for all student self-checkouts.  Due to the large volume of requests on the Friday before a holiday break this form may be unavailable and students will have to be checked out by a parent/guardian.  If possible, make arrangements prior to the planned absence to allow the attendance office to process the request.  If a student is sick, they will need to visit the nurse prior to checking out.  

    Check-Out/Parent Pickup

    Students who are checking-out and being picked up by a parent/guardian are not required to have a note from the parent unless they want to provide a note to excuse the absence.  Notes for check-outs are not accepted via email.  The student will be called from the classroom when the parent arrives at the attendance office.  A government issued ID is required for all check-outs.  

    College Visit Forms

    College Visit Forms are to be completed by juniors and seniors prior to the planned visit.  Forms can be picked up and returned to the attendance office.  

    Excused Absences According to the State Board of Education Rule 160-5-1.10

    • Personal illness
    • Serious illness or death in the immediate family (mother, father, brother, sister, grandmother, grandfather, or guardians)
    • Court Order
    • Religious Holiday
    • Instances in which attendance could be hazardous as determined by the Fayette County School System- School Cancelations
    • Up to five (5) days excused absences per year for students whose parents are in the military and are being deployed or are on leave.
    • Service as a page in the legislature
    • Absences to vote in an election

    *Please note that the State Department of Education determines which incidents are excused and unexcused under Rule 160-5-1.10 (see above).  Family emergencies are not one of the excused absences unless they are listed above.
