The cost of a yearbook is $55.  Don’t wait to order!

You may place a yearbook order one of two ways:

• Order online with the school order #16550, or
  
  
• Print this form and return it to the front office or homeroom teacher with your payment in cash or a check made payable to Flat Rock Middle School.  You may also pay on My School Bucks with a credit or debit card.
