    Privit - Online forms/physical submission

    Fayette County High School uses Privit for the submission of all physical and required consent forms.  Access Privit here.  Instructions on how to register for an account and submit your forms electronically are here.   

     

    Tiger Fan Passes
    Tiger Fan Passes allow FCHS students entry into all home, regular season FCHS Athletic events for one price of $65. The Tiger Fan Pass order form can be downloaded here, filled out at home, and brought to the downstairs Assistant Principal's office before or after school. Order forms are available in the downstairs Assistant Principal's office during the school day. The pass is over a $200 value.

    Fayette County High School Athletics encourages you to see your Primary Care Physician for a sports physical.  Physicals are most effective when your family physician can use your answers to the physical's questionnaire along with his or her personal experience with your family to best determine your ability to participate safely.  Physical forms are below:
