Important! The student application is a fillable PDF document where applicants can type directly into the form, but the fillable feature is ONLY available if you download the file and open it in Adobe Acrobat, as opposed to your web browser’s PDF viewer (the answers will be entered too small to read if you do not follow that instruction.) Alternatively, please print the forms and fill out by hand.

Income eligibility documentation MUST be included with the completed application or it will not be considered. Examples of acceptable types of documentation include: Free & Reduced lunch eligibility letter; SNAP or TANF Eligibility letter, etc.

HOW TO SUBMIT COMPLETED APPLICATIONS

NOTE: For the security of your information, we do NOT recommend emailing the completed application.

To submit the completed application form (.pdf format) and any required documentation electronically, please use this link https://forms.gle/SKMJAs67oAAKS2yWA in which you will be prompted to attach the completed application documents into a REACHGa Google form.

To submit printed copies of completed applications and required materials, please deliver or mail it to Virginia Gibbs, REACHGa Coordinator, LaFayette Educational Center, Building B, 205 LaFayette Ave, Fayetteville, GA 30214

OR

Return it to your student's School Counselor.
