The Critical Illness Plan is designed to pay you a lump sum benefit immediately if you are diagnosed with certain major illnesses. 

Major illnesses covered:

• Cancer
• Heart Attack
• End-Stage Renal
• Stroke
• Permanent Paralysis
• Alzheimer’s Disease

Pays a Lump Sum benefit immediately upon diagnosis

Choose from $5,000 - $10,000 - $20,000 - $30,000 - $40,000

Rates lock-in at enrollment age

Plan will pay a “wellness benefit” of $50 each year for completing a health screening

Guaranteed issue coverage – however, initial diagnosis must occur while covered
